Senior Citizen and PWD Discount Availment Form

To avail of the 20% discount for eligible senior citizen and PWD customers, please follow these steps:

  1. Complete and submit the form below.
  2. Place your order on https://store.mediko.ph/ using the same name indicated on the form. 
  3. Pay for your order in full and wait for it to be delivered. 
  4. A Mediko team member will reach out to you about your SC/PWD discount via call or email.
  5. Once verified, your SC/PWD discount will be refunded via bank transfer or GCash.

Upload ID Photos (Front & Back) Up to 2 files · JPG / PNG / WEBP · Max 5 MB each

Please note that all submissions are subject to verification. Forms submitted on weekends or holidays will be reviewed on the next working day. 

Discounts cannot be combined. If you qualify for multiple discounts or promotions, only the highest applicable discount will be applied to your order.
